Enrollment Agreement/Student Contract Guidelines
The Enrollment Agreement is a contract between the institution and the student, provided to students before enrollment. See Minimum Standard 4.
School and Program Information
- Name and contact information of the institution
- Program name
- Length of the program (weeks/hours)
- Externship/clinical/practicum hours and placement information, if required
- Schedule of classes (days/hours) or spaces to add them
- Start and End Date or spaces to add them
Student Information
- Space for the student's name
- Space for the student's date of birth
- Space for the student’s contact Information
Tuition and Fees – See Minimum Standard 12
- Total cost of program
- Tuition price for the program
- Itemized list of other fees for the program, with any nonrefundable noted as such
- Application fee listed as nonrefundable but no greater than $150 (if applicable) - Unaccredited cannot use other terms such as "registration fee” for this type of fee
- Payment methods available or space for the method used
School Policies
- Career prep services provided and a disclaimer that job placement is not guaranteed
- Attendance policy- maximum allowed absences, and if applicable, options for make-up work, options for students who are failing or have exceeded the allowed absences
- Refund and/or cancellation policy that meets Minimum Standard 12 and Minimum Standard 13 and DOES NOT contain a fixed refund table
- Complaint policy that meets Minimum Standard 14 and includes a link to this GNPEC page: “https://gnpec.georgia.gov/student-resources/complaints-against-institution”
- Licensure/Certification requirements for employment OR disclaimer that students must complete national certification requirements to be “certified,” etc., if applicable
- ONLY SCHOOLS OFFERING A PROGRAM EXEMPT BY ANOTHER GA AGENCY - Disclosure that explains to students that “If you're a student in the (exempt program name(s)), you need to contact (State Agency name) to file complaints that weren’t resolved by your institution or to request your transcript if the institution or program closes.”
